The transition from pediatric to adult care is not just a change of providers but a shift in how your healthcare is managed. This process happens over time and with the support of your Gillette care team. As you work toward a successful transition, Gillette will help you prepare for and participate in your adult care appointments.

 

Preparing for Your Visit with an Adult Provider

Here are some things to consider as you prepare to meet with an adult provider.

Location and Hours

Think about the clinic's hours and how you'll get there (transportation).

Patient Portal

Learn how to use the MyGillette Patient Portal to view your medical records and clinic notes.

Authorized Release of Information

If your provider is outside of Gillette, submit two Authorized Releases of Information (ROIs) to Gillette medical records:

  1.  An ROI to fax the Pediatric to Adult Transition Package to your new provider.
  2. An ROI to send you a copy of the Pediatric to Adult Transition Package.

What to Bring to Your First Visit

  • Health insurance information and a form of payment.
  • A copy of your Pediatric to Adult Transition Package
  • An updated list of your current providers and medications
  • If needed: bring emergency care plans, legal decision-maker documents, or care guidelines for a rare diagnosis.
 

Questions to Ask Your New Provider

Here are some questions to ask when you meet with a new provider. If you are a parent, guardian, or caregiver, you can adjust the language to ask for the patient. We know transition is complex, and you are unique. These questions are examples that may or may not apply to your unique situation. 

  • Do you see adults with disabilities?
  • What information do you need to care for my specific condition? How often will you see me? Can you allow longer appointments?
  • How can I communicate with you between visits (phone numbers, fax, portal, emails)?
    • How do you handle follow-up with test results or patient requests?
    • When receiving a medication refill request, how long does it take to be completed?
  • How will you work with my parents/caregivers/supports?
  • Do you offer care coordination services like nurse care managers or social workers?
  • Are you affiliated with a particular hospital? Do you see patients in the hospital or work with the hospital if I am admitted?
    • Does your Electronic Health Record (EHR) communicate with other hospitals/clinics?
  • How does your clinic work with home nursing agencies and medical supply vendors (phone numbers, fax, emails)?
    • Can you order the equipment or services I need at home?
 

Questions to Ask Yourself After the Visit

After your visit with a new adult care provider, take some time to think about how you felt about the visit.

  • Do the provider and clinic staff treat me with respect?
  • Does the provider listen to my questions and take time to answer them?
  • Does the provider explain things in a way I understand?
  • Is the clinic easy for me to get to?
  • Is there any information that I didn’t receive that I wish I could have? (accessibility, missing documents or information, etc.)
