As you transition from pediatric to adult healthcare, you are not alone! Your healthcare transition team at Gillette will help you make informed decisions about your medical care and the healthcare transition process. Your transition team at Gillette will work with your transition team outside of Gillette, including other providers, school teams, county service teams, and your family.
Who is on my healthcare transition team?
Your healthcare transition team may include a variety of providers and therapists across multiple departments across Gillette:
Your Current Care Team
Your Gillette care team has been with you on your journey. Transitioning to adulthood is no different. Your care team is here to help with the transition to adult care, which can sometimes feel overwhelming. Talk to your team when you are in the clinic for more information on making this a smooth transition.
Rehabilitation Therapists
Your rehabilitation team can include occupational, physical, and/or speech therapy. They will work with you to increase your skills in the areas that matter most to you. The primary goal of the team is to support you to be as independent and comfortable as possible. You can ask to set transition-specific goals, as you work on activity independence or other needs that you might have. Ask to talk with the rehabilitation therapies team when you are in clinic.
Social Workers
Gillette social workers specialize in helping families through the psychological and social effects of managing health care needs. This can include helping our patients and parents adjust to new diagnoses or injuries, coordinating care, coping with life and health transitions, and locating community resources. Talk to your provider when you are in clinic for more information on working with Gillette social workers.
Psychologists and Neuropsychologists
Gillette psychologists and neuropsychologists provide evaluations to help with college and vocational school accommodations and testing to support planning for legal/medical decision-making in adulthood. You can start by talking with a member of your current care team.
Psychotherapists
It’s wonderful to have someone to talk to about things and talking with your family is a great place to start. Sometimes a Gillette psychotherapist can help sort things out and put things into perspective. It’s always okay to ask for help. Talk to a member of your current care team for more information.
Therapeutic Recreation Specialists
Therapeutic recreation helps individuals with disabilities get involved in recreational and leisure activities. The goal of therapeutic recreation is to help all people—regardless of physical or cognitive abilities and limitations—enjoy theirlifestyle. While transition may involve new or different leisure or social interests, therapeutic recreation can help identify and prioritize what is most important to you.
Complex Care Team
Sometimes, you may need a provider focused on your transition to adulthood. The Complex Care clinic team can help develop a comprehensive care plan and walk beside you.
