Moving From Pediatric to Adult Health Care
What is Pediatric to Adult Health Care Transition (HCT)?
Healthcare transition (HCT) requires moving from a pediatric, family-focused model of care to an adult, patient-focused model of care. In Minnesota pediatric health systems, this transition happens between the ages of 18 and 26 years, and changes include, but are not limited to:
- Finding providers, hospitals, and home care services that work with adults and are the right fit for you
- Developing plans of care that help new adult providers understand your unique health care needs
- Preparing for changes in insurance eligibility and understanding options for managing health care decisions such as supportive decision-making or guardianship
