Gillette Children’s Health Services Research program, in collaboration with The National Alliance to Advance Adolescent Health and Got Transition®, received another round of funding from the Minnesota Department of Health (MDH). The Children and Youth with Special Health Needs (CYSHN) section of MDH will continue to support the Health Care Transition Learning Collaborative (HCT LC) through December 2027.
Initially launched in May 2023, HCT LC efforts throughout the past 15 months have shown that clinicians want to learn how to support their patients and families with a safe healthcare transition (HCT) period but lack system-level tools and support.
The goal for the next 3.5 years of the HCT LC is to promote clinical, organizational, and system-wide improvements. All improvements will be consistent with professional recommendations and aim to increase access to recommended HCT supports for Minnesota youth and young adults with complex health needs and disabilities and their families.
The first step in reaching this goal is a strategic partnership analysis of Minnesota organizations (professional, health system, and health plan) to identify new opportunities for HCT education, quality improvement, and systems change. The Health Services Research and Got Transition teams hope to complete this first-stage analysis by Spring 2025.
