Thank you for scheduling your new appointment with Gillette!  

To provide you the best care and most productive first visit, Gillette specialists review relevant records from your other healthcare encounters.    

• Please complete the Records Collection form below to identify which records are needed 
  • Records can be uploaded directly via the form 
  • Or contact your other provider(s) and clinic(s) to send these records to Gillette 
• Requested records must be received 7 days before your visit to allow time for your provider to review them. 
  • If Gillette has not received the records by then, your appointment may need to be rescheduled. 

 NOTE: This form may take 10 – 15 minutes to complete, depending on the number of records needed.
